This Summary is provided to inform eligible Cloaked Users of the benefits under the Master Policy referenced below. This Summary does not state all the terms, conditions, and exclusions of the Policy. Your benefits are subject to terms, conditions, and exclusions of the Master Policy, even if they are not mentioned in this Summary. A complete copy of the Policy can be provided upon request.
The Master Policy for Personal Internet Identity Coverage has been issued to: Cloaked, Inc. (the “Master Policy Holder”), Policy Number: 16452540 underwritten by American International Group, Inc. (“Insurer”), or its insurance company subsidiaries or affiliates. Cloaked is not an insurance company or insurance broker

Limit(s) Of Insurance
Insurance pay out cannot exceed the Aggregate Limit of Insurance above. Legal fees and private investigator fees are subject to prior approval of the Insurer. All Legal Costs shall be part of and subject to the Aggregate Limit of Insurance. LEGAL COSTS ARE PART OF, AND NOT IN ADDITION TO, THE INSURANCE LIMIT.
The Lost Wages, Travel Expense, Elder Care/Child Care, and Certified Public Accountant Costs Limits of Insurance shown above are sub-limits of the Aggregate Limit of Insurance and the most the Insurer shall pay you for those costs.
Filing A Claim

This is a group master policy issued to Cloaked, Inc. If this Master Policy is terminated, your benefits will cease effective that date. The Master Policyholder will inform you of any termination of the Master Policy.
Benefits
The Insurer shall pay you for the following losses incurred as a result of a Stolen Identity Event:

B. Lost Wages
Actual lost wages earned in the United States, whether partial or whole days, for time reasonably and necessarily taken off work and away from your work premises solely as a result of your efforts to amend or rectify records as to your true name or identity as a result of a Stolen Identity Event. Actual lost wages includes remuneration for vacation days, discretionary days, floating holidays, and paid personal days and excludes business interruption or future earnings of a self-employed professional. Computation of lost wages for self-employed professionals must be supported by and will be based on prior year tax returns. Coverage is limited to wages lost within twelve months after your discovery of a Stolen Identity Event.
For purposes of the preceding paragraph, “wages earned in the United States” shall include wages paid to military and governmental employees working outside the United States who are employed by the United States and paid in US dollars.
C. Legal Defense Fees and Expenses
Reasonable and necessary fees and expenses incurred in the United States by you with our consent for an attorney approved by the Insurer for:
D. Unauthorized Loss of Funds Reimbursement
The principal amount of money, exclusive of interest and fees, incurred by you and caused by an Unauthorized Loss of Funds Event first occurring during the policy period for which you have sought reimbursement from the Outside Entity which issued the Access Device and holds the Account from which funds were stolen, and for which you have not received reimbursement from any source.
Definitions
“Account” means a demand deposit (checking), savings, cash portion of a brokerage account, credit card, loan or other account held in your name or under your Social Security number directly or indirectly by an Outside Entity and established primarily for personal, family or household purposes.
“Access Device” means a card, code or other mean of access to an Account of yours, or any combination thereof, that may be used by you to initiate Fund Transfers.
“Fund Transfer” means any transfer of funds that is initiated in person or through an electronic terminal, telephone, computer, or magnetic tape for the purpose of ordering, instructing or authorizing an Outside Entity to debit or credit your Account. Fund transfer includes but is not limited to (i) point-of-sale transfers; (ii) automated teller machine transfers; (iii) direct deposits or withdrawals of funds; (iv) transfers initiated by telephone; (v) forged checks (vi) transfers resulting from debit card transactions, whether or not initiated through an electronic terminal.
“Outside Entity” means a bank, savings, association, credit union, or any other person or business that directly or indirectly holds an Account belonging to an Insured, or that issues an Access Device and agrees with an Insured to provide Fund Transfer services.
“Stolen Identity Event” means the fraudulent use of your name, address, Social Security number, bank or credit card account number or other personally identifying information or other method of identifying you. This includes, but is not limited to, the fraudulent use of your personal identity to establish credit accounts, secure loans, enter into contracts or commit crimes. Stolen identity event shall include Medical identity theft. Medical Identity Theft means the theft of the insured’s personal or health insurance information to obtain medical treatment, pharmaceutical services or medical insurance coverage. Medical identity theft also means the theft of the insured’s personal or health insurance information to submit false claims for medical services or goods.
A “Stolen Identity Event” does not include the theft or unauthorized or illegal use of your business name, d/b/a or any other method of identifying your business activity.
“Unauthorized Loss” of Funds Event means a Fund Transfer from your Account initiated by a person other than you without the actual authority to initiate such transfer and from which you receive no benefit. Unauthorized Loss of Funds Event does not include a Fund Transfer initiated: 1) by a person to whom you knowingly furnished the Access Device to your Account, unless you have notified the Outside Entity that transfers by such person are no longer authorized; 2) with fraudulent intent by you or any person acting in concert with you; 3) by the Outside Entity or its employee; or 4) from any business or commercial account.
Coverage Scope
Stolen Identity Event
The Master Policy provides benefits to you only if you report a stolen identity event to the Insurer by the contact number stated above as soon as you become aware of a stolen identity event, in no event later than 90 days after the stolen identity event is discovered by you and you follow the instructions given to you in a claims kits that you will be provided. These instructions will include notifying major credit bureaus, the Federal Trade Commission’s Identity Theft Hotline and appropriate law enforcement authorities. This claims kit will also instruct you how to file for benefits under the policy if the stolen identity event results in losses covered under the policy.
You will only be covered if a stolen identity event is reported to the Insurer within 90 days of discovery. You will not be covered if the stolen identity event first occurs after termination of the master policy or termination of your membership in the Master Policyholder’s program.
Unauthorized Electronic Fund Transfer
The Master Policy provides benefits to you only if you report an Unauthorized Electronic Fund Transfer to the Insurer at the contact number stated above as soon as you become aware of a Unauthorized Electronic Fund Transfer, but in no event later than 90 days after the Unauthorized Electronic Fund Transfer is discovered by you.
You will only be covered if an Unauthorized Fund Transfer: (1) first occurs while you are a member of the Master Policyholder’s Membership Program, and (2) is reported to the Insurer within ninety (90) days after your discovery of the Unauthorized Fund Transfer. You will not be covered if the Unauthorized Fund Transfer first occurs after expiration or termination of the Master Policy or termination of your membership in the Master Policyholder’s Membership Program.
In addition, you must comply with the identity theft recovery procedures including but not limited to calling the Insurer at the customer service number posted on our websites or provided in your materials to advise that you believe you have become a victim, filing a police report, and must follow the instructions given to you in a claims kits that you will be provided. These instructions will include notifying major credit bureaus, the Federal Trade Commission’s Identity Theft Hotline and appropriate law enforcement authorities. This claims kit will also instruct you how to file for benefits under the policy if the Stolen Identity Event or an Unauthorized Loss of Funds Event results in losses covered under the Master Policy.
Other Insurance
Any payments under this insurance from the Insurer shall be excess over any other insurance, including, without limitation, homeowner’s or renter’s insurance. If you have other insurance that applies to a loss under this policy, the other insurance shall pay first. This policy applies to the amount of loss that is in excess of the Limit of Insurance of your other insurance. In no event shall the Insurer pay more than our Limits of Insurance as shown above.
Duplicate Coverages
Should you be enrolled in more than one Membership Program insured by the Insurer, or any of our affiliates, the Insurer will reimburse you under each Membership Program:
